Introduction

The Hepatitis C Council of South Australia (HCCSA) aims to provide services that are appropriate and of a high standard. Your feedback will help us evaluate and improve our services to you.

The survey takes roughly 5-20 minutes to complete depending on how much you choose to answer and is divided into two parts:

1. Non-identifying information about you;
2. Feedback about the council and/or feedback about particular services the council provides.

Your response will remain anonymous. At no point in this survey will we collect identifying information about you.
